- Growing and innovative Direct Primary Care Model; Monthly subscription covers office visits and routine labs reducing overhead and allowing more time with patients

- Responsibilities:
- Clinic hours: M-F 8a – 4:30p with flexibility to expand patient access
- Call schedule requirement; low-volume call all taken over the phone
- Excellent communication (written and verbal), analytical, and problem-solving skills
- Ability to work as a team member to improve the health of the people and communities we serve
- Provide the best outcome for every patient every time
- Qualifications:
- MD or DO
- Board Certified
- Hold an unrestricted license to practice medicine in Illinois
- Eligible to work in the United States
- Ability to work as a team member
